John Paul II Center for Special Learning Adult Program is open to all individuals 18 years and over, regardless of race, creed, and religion. Admission is not limited to Berks County residents.

An individual seeking admission must be eligible for waiver services through the Office of Developmental Programs and referred by a Supports Coordinator. The referral will include the individual support plan, (ISP), medical and behavioral information. This information will help to determine the ability to provide services, care and support based on individual needs as outlined in the ISP.

If John Paul II Center Adult Program can meet an individual’s needs as outlined in the ISP, the JPII placement criteria will used to determine what needs group and staffing ratio will best meet individual needs.

If an opening is available in the identified group, a plan for admission will be developed. If an opening is not available, the individual will be added to the waiting list for the appropriate group. John Paul II Center alumni are entitled to preferential placement in the Adult Program, as a benefit and extension of the school- based program.

When an opening is available the individual and team will tour the facility, and a team meeting will be held to discuss expectations, care and support needs. If the team agrees the placement is appropriate an admission date will be scheduled.

Prior to the start date the following items must be received:

  • Completed admission forms
  • A physical completed within the last (12) twelve months. Regulations also require a PPD test prior to admission and documentation of a tetanus shot within the last 10 years
  • If necessary, medical protocols will be established, reviewed and signed by individual’s physician
  • Service authorization is approved and present in the Promise system

Fees are determined by the Office of Developmental Services and services are billed to Medicaid through the state Promise system. Individual Service Plans identify the billable services for each Provider. As a pay for service provider the program is only paid when an individual attends and staffing is based on ratios. Excessive absences may lead to discontinuation of services.

All individuals will be accepted on a contingency basis to ensure all needs can be successfully met and a person-centered plan can be developed.
